Job Description

Delivers care to consumers utilizing the nursing process of evaluation, planning, intervention and implementation. Effectively interacts with consumers, families and other ID team members while maintaining standards of professional nursing.. Under minimal supervision, performs professional nursing services; administers the consumers daily medication, acts as a decision maker when an injury occurs to a consumer. Must posses excellent attention to detail for documentation purposes as well as detecting changes in consumer behavior relating to health and wellness.

Skills / Requirements

Valid License as an LVN in the state of California.

Maintains current CPR/ACLS/BCLS Certifications for Healthcare Professionals.

Valid CA Driver's License and personal vehicle to drive between consumer's homes and Health Services required.

One year experience providing services to persons with developmental, learning or physical disabilities preferred.

Must maintain confidentiality of consumer PHI (Protected Health Information) used for treatment, payment, health care, etc.

Access to restricted PHI (Protected Health Information) limited to consumers under direct care only.

Computer literate and keyboard skilled • Excellent communication and interpersonal skills.
